Physical Information

Sports Physicals

All physicals for the 2025/26 seasons must be completed and turned in on Final Forms. No paper copy will be accepted in person. The form WILL NOT be valid without a doctor's signature and appropriate date. To be valid for the 2025/26 school year, the sports physical must be dated AFTER April 15, 2025.
