Many of our commonly used files and forms are available below.  Some are links to electronic forms, others are printable PDFs.

  • FIELD TRIP PERMISSION SLIP
    Permission for the student to fully participate in the school-sponsored field trip. 
  • ICHAT
    The District requires all volunteers to fill out an electronic ICHAT form in advance.  Volunteers must submit a new ICHAT form every school year.
  • MEDICATION AUTHORIZATION (PDF)
    A completed Medication Authorization form must be on file in order for a student to use any type of over-the-counter or prescription medication at school.
  • PARENT EMERGENCY GUIDE (PDF)
    This publication contains important information regarding emergency operation plans. (PDF)
  • PARENTCONNECTION
    ParentConnection allows parents to view information on various aspects of their child(ren)'s student information, including
    • Attendance records
    • Assignments/grades
    • Transportation schedule

  • SPORT PHYSICAL (PDF)
    A completed Sport Physical must be on file in order for a student to participate in a team sport before the first practice/try-out for that sport.  Please print both pages and read the entire form.
